Online Health Form: All health history forms will be submitted using the online CampDoc system. This system allows us to better protect your campers private information and allows for better communication between parents and camp staff regarding your campers specific health needs. After registering for camp, you will receive a separate email from CampDoc with a link to the online form. This email will arrive 1-2 weeks after you submit a camp registration. If you do not receive an email, please contact the camp office at campnanabosho@ymcafoxcities.org. The online health form must be completed no later than two weeks prior to the start of your camper's session.
